Deficiency is common and easy to miss: it climbs with age, with acid-reducing medication, with metformin, and with any diet that’s light on animal foods. Methylcobalamin is the bioactive form your body can use directly, unlike cyanocobalamin, which needs to be converted first. If you’re tired, foggy, or your mood feels harder to hold steady than it should, B12 status is worth checking before you assume it’s “just hormones.”
Why B12 matters for hormones, not just energy
B12’s reputation is “gives you energy,” which undersells what it actually does. - Gut conditions (Crohn’s, celiac, IBS, prior gastric surgery) — impair the absorption step regardless of intake
- Pregnancy and breastfeeding — increased demand, worth discussing with your OB
If more than one of these applies to you, it’s worth testing rather than guessing — B12 deficiency is one of the more fixable causes of fatigue and brain fog, but only once you know it’s actually the issue.
Methylcobalamin vs cyanocobalamin — the form matters
Not all B12 supplements are equal, and this is the part most labels don’t explain clearly.

If you’re supplementing, methylcobalamin — ideally sublingual, since it bypasses some of the stomach-acid-dependent absorption step — is the form I’d point a friend toward. Look for 1,000mcg as a common daily maintenance dose, and don’t be alarmed by high numbers on the label: B12 has a wide margin of safety and excess is simply excreted.

Two to three servings of fatty fish a week — the same target from my omega-3 guide — comfortably covers B12 for most women who aren’t dealing with an absorption issue. If you are dealing with one of the risk factors above, food alone often isn’t enough regardless of how much you eat, since the bottleneck is absorption, not intake.
What to test (serum B12 alone isn’t enough)
Serum B12 Can Look “Normal” and Still Be Functionally Low
A standard serum B12 test measures total B12 in your blood, including inactive forms your cells can’t actually use. This means you can test “in range” and still be functionally deficient at the cellular level. Two additional markers catch what serum B12 misses:
- MMA (methylmalonic acid): rises when cells are truly B12-deficient, even if serum B12 looks fine
- Homocysteine: elevated when methylation is struggling — B12, folate, and B6 all feed into keeping this number down
If your serum B12 is “normal” but you still have symptoms, ask your doctor about adding MMA and homocysteine before ruling B12 out entirely.
